By providing a telephone number to Quest Health RPM Services, you agree that a representative of Quest Health RPM Services, or a third party acting on its behalf, can contact you at the number you provide, potentially using automated technology (including texts/SMS messaging) or a pre-recorded message. Your consent is not an obligation to receive any of our products and/or services. For SMS text messages, you may remove your information by replying “STOP” to the SMS text message you have received, and we will remove your personal information within 10 days of receiving such request.

Usage Data We may collect information on how the Service is accessed and used (“Usage Data”) by you. This Usage Data may include information such as your device’s Internet Protocol address, browser type, browser version, the webpages of our Site that you visit, the time and date of your visit, the time spent on those webpages, unique device identifiers and other diagnostic data. Generally, we do not consider Usage Data to be personal information because Usage Data by itself usually does not identify an individual.

We do not track our users across time or across websites.

We also may share information that we collect, as needed, to enforce our rights, protect the rights, property or safety of others, or as needed to support external auditing, compliance and corporate governance functions. This includes reporting to the Food & Drug Administration or other government bodies as required to report adverse events or otherwise comply with the law. We may disclose personal information, including sensitive personal information as permitted, as we deem necessary to respond to a subpoena, regulation, binding order of a data protection agency, legal process, governmental request or other legal or regulatory process. We may also share personal information as required to pursue available remedies or limit damages we may sustain.

We may share information for public health purposes, including reports to public health agencies or legal authorities charged with preventing or controlling disease, injury or disability and reports to employers for work-related illness or injuries for workplace safety purposes.
