How does RPM work?

1. Meet your care team

Our physician will meet with you for a full health review to understand your goals and help put you on the path to success. From there, you'll be introduced to your dedicated Medical Assistant—a trusted partner who will provide ongoing support and guidance to help you take control of your diabetes with confidence.

2. Receive your device

Measure your blood glucose using our provided Blood Glucose Meter, where your results will be automatically sent to your care team. Need more supplies? Just say the word and we will provide more. Using a continuous glucose monitor? Great! Our team can connect to it for even closer monitoring.

3. Check-in every month

Our care team will reach out once a month - or more often if needed - to review your numbers and offer helpful insights and guidance to help you feel your best.
